Privacy and data-use disclosure
Local Health Archive PoC is an individually developed, local-use application. Its purpose is to retrieve health information that an individual authorizes and store it under that individual’s control.
Information retrieved
Depending on the healthcare organization and the choices made during authorization, the application may retrieve demographics, encounters, clinical notes, test results, medications, conditions, allergies, immunizations, procedures, care plans, claims, and related health records.
Storage and access
Retrieved information and authorization credentials are stored locally on the individual’s device. The application does not operate a remote health-data store. It does not give its operator, advertisers, data brokers, researchers, or other third parties access to retrieved records.
Use and disclosure
Retrieved information is used only to provide record retrieval and local organization requested by the individual. It is not sold, licensed, used for advertising, or disclosed for marketing or independent research.
Retention, export, and deletion
Files remain on the individual’s device until the individual deletes them. Because the files are locally controlled, the individual can inspect, copy, export, or delete the complete locally stored record at any time.
Authorization
The healthcare organization controls its own authentication and consent process. An individual can revoke an authorization through the organization where supported. Local copies already retrieved remain under the individual’s control and can be deleted by that individual.
Security
The application limits requested access to read-only record retrieval and stores credentials and records as owner-access-only files where the operating system supports those permissions.
